A self-assessment form and pertinent information form will be provided to you at the beginning of the year for which your agency or institute of higher learning is scheduled.
The pertinent information form should be completed and returned as soon as possible to expedite communication between the Monitor and the correct representative for your agency.
The self-assessment form should be completed by the deadline given to you by the Monitor at the beginning of the year. Agencies are reviewed on a quarterly schedule. All personnel policies, procedures, or other manuals/documents referenced in your self-assessment should be provided simultaneously.
Additional Required Materials:
- Organizational Chart
- Current FTE (full-time equivalent) List: a sample population will be pulled from this list and their personnel files will be requested.
- Hiring Packets: the three most recent for each of the past three fiscal years (for a potential total of 9).
- Hiring packets should include every step of the process – job posting; all applicants and their application package; interview method, matrix, and scoring for each selected applicant; selection authorization, and; any conditional and final job offers.
